What is wrong with the 0–10 pain scale?

Nothing, for people who can use it. Producing a number is a conversion: notice an internal state, judge its size, compare it against an imagined worst case, and turn all of that into a figure — while in pain. That conversion is a cognitive task, and cognitive tasks get harder under load. For some people it never resolves at all, and the guess is what gets treated. The scale is one instrument among several, and a poor first choice for someone who cannot use it.

Why does the numeric scale fail neurodivergent patients more often?

Several reasons stack up. Interoception — reading what your own body is doing — is measurably different for many autistic and ADHD people, so the internal signal may never arrive as something countable. Literal interpretation takes 'worst pain imaginable' at its word and answers precisely, which reads as stoicism. Under load many people lose access to speech while keeping full awareness. And anyone who learned to make things easier for others will report less than there is. None of it is exclusive — it happens to everyone under enough pain and fear, just less often.

What does the wheel ask instead of a number?

Three things. What the pain is like — twelve qualities such as burning, stabbing, pressing, electric, each with the ordinary words that go with it. What it stops — eight levels written as observable things, from nothing yet to needing help now. And how well the person can currently report it, because someone braced on adrenaline will describe less pain than they have and someone shut down may not be able to locate it at all.

Is this a triage tool?

No. It does not measure pain, rank it, or decide how urgent it is, and it does not replace clinical assessment. It cannot rule anything out. Pain that is sudden, severe or new needs medical attention whatever words end up in the window. It is a way of saying it, which is all it claims to be.

Can it be used to judge whether a patient is exaggerating?

No, and it says so on the tool itself. The third wheel is about the conditions an answer is being given under, not about whether the answer is true. Used properly it tells a clinician what to ask next and when to ask again. Used to discount a report it becomes one more instrument for calling a patient unreliable, which is a label this group already carries too often.

Yes. Six sheets make two cards: one disc printed on both sides for what only the patient can answer, and a separate disc for what the pain stops, which is meant to be handed over. Each wheel needs its own cover because the window has to be exactly one segment wide. Base discs 340 mm, covers 320 mm so the base overhangs as a grip, joined with a split pin. Matte, never gloss.
